How to Register
Registration is completed online. You will be asked a few questions to determine whether the clinic is appropriate for you and to select an available clinic date.

Review the available clinic dates above and choose the clinic that works best for your schedule.
2. Complete the Form
Click the Register button for your selected clinic and answer the questions on that clinic’s online registration form.
3. Watch Your Email
After your registration is reviewed, you will receive an email with additional information about your registration and next steps.
